To provide our services, we may collect:

Personal Information

• Name, address, telephone number and email address
• Date of birth
• Emergency contact details (where appropriate)
• Appointment, booking and payment information

This information is collected to administer bookings, provide our services and process invoices where required.

Special Category (Health) Data

To provide safe and appropriate care, we may collect health information including:

• Medical history
• Family medical history
• Current medication and allergies
• Lifestyle information, including nutrition, exercise, alcohol consumption and smoking status
• Sleep, stress and mental wellbeing information
• Women's or men's health information where relevant
• Occupational Health information where applicable

During your appointment we may also collect clinical measurements and diagnostic results including:

• Blood pressure
• Resting heart rate
• Body composition measurements
• Cholesterol profile
• Blood glucose
• HbA1c
• Vitamin D
• Ferritin
• MXA CRP
• PSA (where appropriate)
• Kidney and liver function tests
• Urinalysis
• ECG recordings
• Other health assessment findings relevant to your appointment

Your information is collected to:

• Deliver your requested health assessment or therapeutic service
• Produce personalised reports and recommendations
• Monitor changes in your health over time
• Support Occupational Health assessments where applicable
• Make referrals to other healthcare professionals where appropriate and with your consent
• Meet our legal and professional obligations

We process your personal information under the following lawful bases:

• Performance of a contract
• Legitimate interests
• Legal obligations
• Explicit consent where required for processing health information

Following your appointment, your report will be provided securely via password-protected electronic delivery.

With your consent, we may retain your report and assessment data for up to two years to allow comparison during future assessments and to support continuity of care.

If you choose not to consent to storage beyond the completion of your appointment, your report will be securely deleted once the service has been completed and sufficient time has been allowed for delivery or resending if required.

You may withdraw your consent at any time by contacting The Wellbeing Co.

We take the security of your information seriously.

Your information is stored using secure, password-protected and encrypted systems with appropriate physical and electronic safeguards designed to prevent unauthorised access, disclosure, alteration or loss.

Where reports are retained for future comparison, they may be pseudonymised to provide an additional level of protection.

Your information is treated as strictly confidential.

We will never sell your personal information.

Information will only be shared:

• With your explicit consent
• Where necessary for referral to another healthcare professional involved in your care
• Where required by law

Under the Data Protection (Jersey) Law 2018 you have the right to:

• Be informed
• Access your personal information
• Request correction of inaccurate information
• Request erasure where applicable
• Restrict processing
• Request data portability
• Object to processing
• Challenge automated decision-making where applicable
